Request Help
All individuals seeking assistance through the AHA-CAP must be residents of Monmouth County and have a combined gross income from all sources that is at or below 200% of the federal poverty limit. To see the current income guidelines please click the arrow on the right.
In order to assist you in expediting your request for assistance we have provided three simple steps to completing your application. You may submit your documentation electronically, through email or facsimile, or by mailing or dropping of the requested information.

Passport
New Jersey Driver’s License
State Photo ID
Monmouth County Photo ID
Social Security Cards
Birth Certificates
Proof of income for all household Members:
Most recent paystubs covering the last three months if working
Current award letter for any Social Security, Pension or Disability
LOOPS letter from local unemployment office if receiving Unemployment
Affidavit of no income
A certified copy of your most recent federal tax return if self-employed

For Rental Assistance:
All individuals seeking rental assistance must submit the Rental Verification Form to their landlords for verification of rent and rental arrears. Additionally, a copy of the most current lease, rent ledger and rental subsidy documentation is also requested. Should your landlord not have a ledger available, the linked Tenant Rental Ledger may be used.


For Mortgage Assistance:
All individuals seeking mortgage assistance must submit the Mortgage Verification Form, along with a copy of the most current Mortgage Statement.

For Utility Assistance:
Please provide the most recent utility bill.
For Transportation Assistance:
Please read, complete , sign and submit the Taxi Voucher Application Form for each person requesting transportation.

For Youth Services:
Please submit the most recent report card for each child
